MPHC HandOut Request Page

You may request handouts for any workshop or videostream program sponsored by MPHC (provided a handout exists).  Fill out the form directly below these instructions.  Make sure you fill in all of the blanks.  Enter None in the blanks that do not pertain to you - For Example, if you do not have a fax machine, then enter None in the blank.  When you have completed filling in the blanks, click on the submit button at the bottom of the page.  A confirmation page will be sent back to you confirming that the data was sent to MPHC.  We will send you the requested handout via e-mail first, fax if e-mail is not an option or doesn't work and finally by mail if the first two are not options or have problems.
